Prevention Case Manager

Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to protecting the health and welfare of drug users and sex workers. Using a harm reduction approach, PPP offers culturally sensitive, non-judgmental services to address the health and social service needs of drug and hormone users and sex workers in Philadelphia.
The Trans-health Information Project (TIP) is the product of a collaboration between PPP and GALAEI (Gay and Lesbian Latino AIDS Education Initiative) and provides harm reduction services to everyone on the transgender spectrum. These include team-based outreach and education; referrals and information; condoms and safer sex materials; health care information and assistance in accessing health care; and educational workshops.
The Prevention Case Manager (PCM) targets populations of transgender, transsexual, and gender-variant (TG/TS/GV) people in Philadelphia who are at risk for HIV/Hep-C. Performs a variety of services including individual counseling, case management, and service brokerage on behalf of clients. Provides one-on-one and group centered education on TG/TS/GV health issues, HIV/AIDS, hormone and silicon injection, sex work, substance use, and harm reduction. Links current clients and other members of the target population(s) to appropriate services including HIV testing and counseling, educational and support groups, sterile syringe exchange, trans-specific medical care, legal assistance, public benefits, drug treatment, and other supportive services. Provides support to educational workshops and assists in street-based outreach. Provides services at PPP, GALAEI, the Washington West Project, and other community sites. Keeps detailed records on each case. Reviews and upholds policies and procedures for TIP in accordance with standards set by the AIDS Activities Coordinating Office, Prevention Point Philadelphia, and GALAEI. Must have: Bachelor’s degree; 1-2 years experience in the field of health care and social services. Compensation $26-$29,000 + excellent benefits package including: health, dental & vision insurance; short and long term disability; life insurance; and generous paid time off benefits.

The Social Worker performs a variety of services including individual counseling, case management, public benefits counseling and advocacy and service brokerage on behalf of clients accessing the Street-Side Health Project Clinics and other Prevention Point Philadelphia programs. Counsels and provides guidance to individual clients (and partners or families) regarding social, medical and/or psychiatric issues. Remains current on the variety of insurance and health maintenance options and assists clients in accessing public benefits. Coordinates client linkage to drug treatment, behavioral health care, specialized medical care and social services. Health education and skills building interventions with clients are included in the scope of work.
Must have: Bachelor’s degree, MSW/MA preferred; 1-2 years experience in the field of health care. Bilingual (Spanish) preferred.

Position Summary:
Prevention Point Philadelphia (PPP) is a multi-service public health organization dedicated to protecting the health and welfare of drug users and sex workers. Using a harm reduction approach, PPP offers culturally sensitive, non-judgmental services to address the health, social service and advocacy needs of drug and hormone users and sex workers in Philadelphia. The executive director works collaboratively with the PPP’s board, staff and community stakeholders and oversees a diverse set of programs including the: Syringe Exchange Program, Street-side Health Project, Prevention Services Program, Trans-health Information Project, and Legal Advocacy Project. The executive director will set fundraising priorities, oversees fiscal operations, manage the organization and maintain a leadership position in service delivery, fundraising and advocacy.
Duties and Responsibilities
With the Board of Directors:
• With the board of directors, develop a vision and strategic plan to guide the organization.
• Work with the board of directors to ensure that programs meet the needs of the local communities in which PPP works.
With Staff:
• Ensure, by effective management and leadership, that the day-to-day operations and programs are professionally and efficiently organized and administered.
• Manage and supervise all PPP staff both through his/her own efforts, and administrators and program coordinators under his/her leadership and direction; delegate responsibilities and assignments as appropriate.
• Recruit, hire and release personnel, through his/her own efforts and through the members of the administrative team. Develop and implement professional development curriculum for administrative staff. Create or oversee the creation of comprehensive position descriptions for all personnel; conduct routine and exception staff performance evaluations; counsel, discipline, and recommend terminations of staff who fail to meet the rules, regulations, and requirements for their jobs.
• Maintain a climate which attracts, keeps and motivates a diverse staff of top quality people.
• Develop and implement organizational policies and procedures as necessary.
• Conduct twice monthly staff meetings to disseminate and elicit pertinent information.
Regarding Communications:
• Maintain, promote and provide visibility for PPP by serving as a credible, articulate and effective spokesperson, in person and in writing, to all city agencies, community organizations, foundations, corporations, members of the press, private individuals, and other stakeholders. Reinforce and build positive understanding of and reputation for the work of the organization. Conduct mediation with concerned parties.
• Build effective partnerships with organizations and individuals working in the field and in communities where PPP conducts outreach and provides services; leverage PPP reputation, stature and effectiveness through these efforts.
Regarding Finances:
• Oversee the organization’s financial operations; oversee the budgetary process and ensure that sound financial controls are in place and that funds are spent wisely; work closely with the administrators and finance committee to maintain PPP’s current and future financial and administrative well-being.
• Be responsible for developing and maintaining sound financial practices.
• Work with administrative team, finance committee, and the board in preparing a budget; see that the organization operates within budget guidelines.
• Ensure that adequate funds are available to permit the organization to carry out its work.
• Jointly, with the chair and secretary of the board of directors, conduct official correspondence of the organization, and jointly, with designated officers, execute legal documents.
Regarding Development:
• Working with the administrative team and the board, ensure a sound funding base for the organization; broaden private fundraising so that development activities evolve as the funding environment evolves; create targeted efforts to increase fundraising from all donor segments – foundations, corporations, government, individuals and special events. Write grants. Develop and manage new and existing sources of funding.
• Understand the actual and prospective funding communities for PPP. Create strategies to secure ongoing significant sources of private/unrestricted funds to support PPP. Play an active role in fundraising. Cultivate and solicit key donors, working in partnership with board members when appropriate. Identify innovative methods for tapping new funding sources and networks on behalf of the organization.
• Together with staff, monitor key demographic shifts in the client population and the changing needs of clients, and analyze how such shifts impact programming and fundraising activities; communicate their implications to entire staff and board together with recommended action.
• Ensure compliance with government, funding source, and PPP regulations and requirements, including licensing and certifications, program costs, health and safety, etc.
• Maintain agency files.
Skills Required:
• Demonstrable understanding of and commitment to a harm reduction philosophy and approach in working with individuals and communities
• Knowledge of community organizations, social service programs, and medical providers
• Excellent organizational and time management skills. Ability to manage and prioritize multiple projects and tasks. Ability to demonstrate cultural competency in working with diverse populations.
• Professional oral and written communication skills.
• Demonstrable leadership skills. Ability to build and manage a team.
• Attention to detail and ability to meet deadlines.
Minimum Qualifications:
• Masters degree in Social Work, Public Health, Public Policy or related field. Equivalent experience will be considered for qualified candidates.
• Experience working with communities impacted by HIV/AIDS, substance use, poverty, and/or homelessness.
• Demonstrated commitment to community organizing, advocacy and social justice, particularly as related to HIV/AIDS, gender-justice, poverty, homelessness, and harm reduction.
• Experience in program development and evaluation.
• Experience in grant writing and other fundraising activities.

HIV Counselor/Tester

Must have: at least two years work experience with communities impacted by HIV/AIDS, substance use, poverty, and/or homelessness; direct service experience with one-on-one counseling and group facilitation experience preferred; ability to work Tuesday-Saturday.
